This multi-state geriatric and palliative medical practice treats patients with multiple chronic conditions and frailty who need symptom management and care coordination.   Their physician and nurse practitioner-led care teams work in skilled nursing, assisted living and independent living facilities, as well as senior housing communities and transition to home programs. 

In July 2021, their leadership tapped AK Consultants and Healthcare to launch these services in Florida markets. 

 AK led a number of coordinated initiatives to introduce and scale the practice:

• recruitment of physicians and mid-levels 
• rebranding of palliative care program including development of new collateral, website content and content marketing

Sixteen months later, the practice is established in 40 facilities throughout Florida, representing 2,000 new patients to the practice. Growth projections for year two in Florida are expected to top 100 facilities and 5,000 patients. 

Key to the practice’s accelerated growth is its sales and marketing infrastructure, which was built by AK Consultants and Healthcare.  Starting from scratch, AK focused on integrating a culture of engagement with top talent and proven practices in post-acute care sales.  Based on AK’s model, the practice was able to move its entire sales function in-house and is well-positioned for continued expansion throughout Florida.
